New Grower Urgent help needed

4 out of 7 healthy sprouts died on me.
What could be the reason?
 
My soil is perlite and peat moss.
Could it be under watering? Does under watering cause the stem to become very thin at one point and break?
 
O is it only peat and perlite? Could be a PH problem. It looks like a water issue to me though. Like the little guy is getting dry and shriveling at the base. I wish I had a picture of how close I use to get my CFL lamps. They were very close like a few inches away but they were 26W bulbs. I made sure I had good air flow across the bulbs to cool them and the surface of the bulb base usually was in the mid 70* range. I tried not blowing a fan on my bulbs and turned them off when the temps got to 148* and started to smell like hot electronics. Keeping a light breeze blowing on your bulbs is important. Your bulbs are larger so they will be hotter but even my 250W CFL I can touch the base of the bulb when it is on. Its pretty worm but I can touch the base with out burning my fingers. I put my 250 about 8" away. I would try different soil. Were are you getting your water from?

You think there might be too much drainage so the seedling gets dry real quick? I've kept it moist around the seedling and the leaves didn't show any signs of being dry/under watered.
Yes, it is only peat and perlite.
Tap water left for 24+ hours then pH'd to 6.3-6.4 using lime. Actual lime.
 
? I have never used actual lime before. Your lime could be toasting them.

Lime is a alkaline it raises PH it doesnt drop it.In any case potassium hydroxide and phosphoric/nitric/sulfuric acid for ph up and down respectively.
Acids lower ph,alkalines raise ph

GH or AN Both make very affordable ph up/down supplements.

Got to be honest, I never PH. So I wouldn't know how to use lime in planting. But if it's anything like the 'real lime' I used whilst in construction, I wouldn't want it near a seedling in even minute doses. If you go again, I'd recommend trying a soil mix (I use biobizz allmix) where you can just let them grow for a while. Good luck
