The MG Perlite only has trace amounts of NPK and would not cause what you now have.
Maybe you should ring the nursery you got the soil from and ask them what nutes they put in it and how much...
Also ask what they set the soil mix PH to, if they checked it at all.
The NL looks nute cooked to me even in the early pics, or PH way out....or a combination of both, I have nute cooked and PH burnt babies and they just never came back to anything worth growing.
If your soil mix is stupidly hot from slow release ferts then flushing will help a little but you just cannot get all that slow release shit out, but it may give the plants enough time to come back to living a bit
