Sorry Squid, it's a teaspoon per quart for foliar feeds, a tablespoon per gallon for regular feeds.

Edit: And IME, you don't want to add any more nutes to those feeds since we're just trying to get your ph back up in range. Over nuting can just cause more problems, you can put in all the nutes you want, but if your ph is off then the plant cannot uptake said nutes.
So I would just feed with plain ph water with lime ph'd to 6.8 like gg has stated. Working it into the top can work too. I personally have never done either by the way, but if I did I would add it to the water and go that way, it's up to you. Either is fine I say.
But if you add it to the water do not add 3 Tblsp per Gallon, more like one Tblsp. per Gallon. Lime contains calcium and too much can light your roots up in a bad way.
Then when we get your ph back in range you will notice a difference because it will then be able to start eating the nutes that you have already been feeding it. And when you know it's back up around 6.5 then go back with the feeds. Just my opinions here. Nutes pull your ph down too, keep that in mind. So that's another reason why I wouldn't add any to the water that you're raising your ph with, just wait, it'll be fine because the ph should be back up within a single week.
