Here's the recipe I've used in the past:
4 gallons Pro Mix
1 gallon worm castings
1 gallon perlite
4 tablespoons blood meal
4 tablespoons bone meal
8 tablespoons kelp meal
4 tablespoons epson salts
Dolomite lime or sulfur as needed to adjust the pH.
This makes approximately 6 gallons, enough for two 3 gallon pots. I also add 1/2 teaspoon of mycohorrizal fungi to each pot when I transplant.
I've moved away from this mix and am now using straight, unamended Pro Mix, mainly because I can't get bulk worm castings anymore. I'm now supplying all nutrients through feeding.
