Xtreme Gardening Mykos is pretty popular.

They have good instructions for a number of different grow methods on the bag/website.

Dynomyco is my favorite. I mix a little into my seedling mix immediately below where I am planting my seed. I then tend to sprinkle some more across the top of my soil where I sprinkle my 12 seed cover crop blend and then scratch it all in.
